Your 2025 Benefit Options

Louisiana Machinery is pleased to offer a comprehensive benefits program, that offers you choice and flexibility, so you can take charge of your physical, financial and emotional well-being. Your medical benefit options will include

  • VBP Copay plan through HealthScope with a $2,000 deductible and PHCS network
  • Embedded HDHP with UMR Choice Plus network and $4,000 deductible

If you are enrolled in the VBP with HealthScope and undergoing an elective procedure, please make sure you call the number on the back of your ID card to pre-certify for the procedure.

Louisiana Machinery is continuing their partnership with HealthScope Benefits in offering the Value-Based Payment option.
Value-Based Payments works just like a Classic PPO plan in some regards; members will still only be responsible for copays at primary care physician ($25) and specialist offices ($50), but the Value-Based Payment option has an open network for facilities, which means you can visit any facility, there is no “in-“ or “out-of-network" coverage.

LA Machinery is pleased to continue to offer an HSA qualified, Embedded HDHP (EHDHP) administered by UMR that meet the health care needs of you and your covered dependents.

Administered Companion Life

The Louisiana Machinery Basic Life/AD&D plan is a 100% employer-paid benefit that provides your family with financial protection in the event of your death or the death of a covered dependent (as long as you are enrolled in medical).

The Accidental Death & Dismemberment (AD&D) plan protects you and your family in the event of your death or injury due to a covered accident in addition to the benefit provided by your Life coverage.

Short-Term Disability

LA Machinery’s Short Term Disability plan provides a weekly benefit for expenses incurred if you cannot work several weeks due to an illness or injury. Your doctor must certify that you are unable to return to work on a full-time or partial basis.

LA Machinery’s Long Term Disability plan provides a monthly benefit in the event that you become disabled and are unable to work. Long Term Disability provides replacement income to give you financial security.

Louisiana Machinery is continuing to provide you with a voluntary plan that offers convenient access to legal services you may not be able to afford on your own. This plan is to give access to legal representation for you and your family at only $21.00 a month. The nationwide network includes over 15,000 attorneys.

To continue coverage, terminated and retired employees must apply for portable enrollment within 30 days of their last day of employment by calling (800) GET-MET8. Portable enrollment remains in effect for a 30 month period and is non-renewable. Enrollment is prepaid and the cost is equal to the in force group rate x30.
