Effective 9/1/2025, Transamerica Universal Life will no longer be payroll deducted by Louisiana Machinery. In order for you to keep your policy(s), you will need to complete the necessary form to convert to a direct bill. Watch your email and mail for additional information.

Active Enrollment
Open Enrollment will start Tuesday, July 15th and go through Thursday July 31st. Fidelity Enrollment Services benefit counselors will be available to speak with employees beginning starting Tuesday, July 15th through Thursday, July 31st, 2025. This year, at select locations benefit counselors will be available onsite to assist in the enrollment process.
During these meetings, FES will in-person:
- Review your benefits packages
- Answer questions and provide additional information
- Help you complete your enrollment in your elected benefits

This year is an active enrollment which means you must speak with a benefit counselor to enroll or waive coverage. You can schedule an appointment using the two buttons below or by calling (833) 737-1947 between Monday - Friday 8am - 5pm CDT during the open enrollment period.
Download the list of locations and days of your branch meeting, enrollment for your location will begin on the following business day.

Louisiana Machinery is pleased to offer a comprehensive benefits program, that offers you choice and flexibility, so you can take charge of your physical, financial and emotional well-being. Your medical benefit options will include the VBP copay plan and the Embedded High Deductible Health Plan with the $4,000 deductible.

UMR REMEMBER TO CALL AHEAD
Louisiana Machinery has joined with HealthSCOPE Benefits to bring you a Value-Based Payment plan.
Value-Based Payments works just like a Classic PPO plan in some regards; members will still only be responsible for copays at primary care physician ($25) and specialist offices ($50), but Value-Based Payments does not have a traditional network of facilities like a Classic PPO.
LA Machinery is pleased to continue to offer an HSA qualified, Embedded HDHP (EHDHP) administered by UMR that meet the health care needs of you and your covered dependents.

Administered Companion Life
The Louisiana Machinery Basic Life/AD&D plan is a 100% employer-paid benefit that provides your family with financial protection in the event of your death or the death of a covered dependent (as long as you are enrolled in medical).
The Accidental Death & Dismemberment (AD&D) plan protects you and your family in the event of your death or injury due to a covered accident in addition to the benefit provided by your Life coverage.

The Louisiana Machinery Voluntary Term Life plan allows employees to elect additional life and dependent life coverage to supplement the Basic Life policy. This coverage is entirely optional and is paid for by the employee. Spouse and dependent child coverage is only available if the employee is insured for Voluntary coverage.
Initial enrollees have a “one-time” window, during this Open Enrollment period without any medical questions asked up to the Guarantee Issue limit, subject to policy guidelines.
Current enrollees can elect to enroll in an additional 10k for employees and 5k for spouse without the need for Evidence of Insurability.
Should you decline any of the aforementioned coverages, you will be subject to Evidence of Insurability at a later date.

When you enroll in either of Louisiana Machinery’s High Deductible Health Plans, you have the option to open a Health Savings Account (HSA). HSA funds can be used to pay for eligible medical, dental and vision expenses for you and your eligible dependents, including deductibles, coinsurance, prescriptions, acupuncture and more, tax-free, now and in the future. Family members do not need to be covered under your plan in order to use the HSA funds.
With the HSA, you are in charge. You decide: how much you’ll contribute; when to pay for eligible expenses with HSA funds directly (you can also reimburse yourself from the account); how and if you want to invest your HSA funds (a balance of at least $2,000 is required to invest); and whether to save HSA funds for future expenses or retirement.
